Register for a Library Card

Getting a library card is the best way to access the myriad of materials and resources available through the Southwest La Plata Library District. Please fill out the form below and someone from your selected branch will contact you about how to pick up your card.

When you pick up your library card, please bring a photo ID and proof of address.

Use my library card to check out materials.
Be responsible for all materials on this card.
Pay all fines and fees.
Not lend this card to others.
Let the library know right away if I lose this card or if my contact information changes.
I understand that a minor’s account cannot be accessed without their physical card or card number.
